Project: Gorgon is a 3D fantasy MMORPG (massively-multiplayer online role-playing game) that features an immersive experience that allows the player to forge their own path through exploration and discovery. I think it'd be cool if getting a serious hit or dying had a really tiny chance of giving people different types of synaesthesia.
Like seeing a red smoke kind of effect come off of an enemy when it makes a sound, or different (unattractive) colors coming from them depending on what they do to you. Another type could be visible signs of sound, like arrow flying towards you when anything makes a loud sound, with the signs getting bigger as you get closer.
Also, if someone dies a lot in a short amount of time then it causes a temporary insanity effect, the effect will only happen if you put a question mark at the end of what you say in chat. It will show text pop up on your screen saying "I don't know" or "Not sure, that's why I'm asking", things like that.
Lastly, I think it'd be funny if when a person runs for a long period of time there's a chance they'll trip up for a few seconds.
-Update- Also, for another type of insanity, if you're facing away from water, like a pond or lake, the water creeps towards you a bit, and when you turn to it, it moves back to it's original place swiftly. Kind of like a paranoid delusion effect.
